Patient Care Plan

You’ll want to complete the patient care plan as you are wrapping up your patient visit or, when the visit is finished.

To do this:

  1. Select the Care Plan tab.
    You can complete the care plan in any order you choose however for this example, we’ll start with the Diagnosis.

  2. Locate the Diagnosis section (at the top), and then select Add.
    A list of possible diagnoses display. You can select one of the suggestions from the list or, enter your own into the Diagnosis field.
    Select Add again if you wish to add more than one diagnosis.

  3. To electronically prescribe medications, select Add within the Prescriptions section.

  4. Select Mark as Reviewed to indicate that you have reviewed that either the patient has no known drug allergies (NKDA), or that the patient has a known allergy. (When applicable, Allergies are indicated with a red, warning flag, as shown below.)

     

  5. Select Yes for Prescribe in console.

  6. Enter the name of the medication you wish to prescribe within the Medication or Supply field, and then select the preferred medication and formulation from the drop-down menu.

  7. Enter the labeling information into the SIG field, along with the Dispense instructions and Refill information.

NOTE: The SIG field is not a free text field to ensure that patients are prescribed appropriate dosages. (See Maximum Dosages for more information.)

You may receive Details Available or Actions Required advisories and warnings within the Prescription boxes. See Medications and Dosages for more information on what these mean, and how to proceed.

  1. Complete the remaining sections (Referrals, Labs, etc.) of the care plan, and then select Preview Care Plan. (See Documents to learn more about creating Medical Recommendation Letters.)

  1. Preview the care plan, and then select Send Care Plan to send the care plan to the patient within the 98point6 app.

  1. Select Sign and Finalize to complete the visit.
