Behavioral Health - Application

The following information will help guide you through the Behavioral Health appointment and visit process. It is important to note that you must first complete patient registration (onboarding) through the 98point6 app.

Behavioral Health appointments are available to adults on a referral base only. As a result, you must first complete a primary care visit through the 98point6 app, and then receive a referral by your clinician.

To begin the process:

1. Complete patient registration (onboarding). See Patient Registration if you have not yet registered your account within the app or have not yet completed a primary care visit.

2. (Adults only) See Patient Primary Care Visit to complete your primary care visit and to receive a referral (if you have not already done so).
